Application Form
Applications should be submitted as soon as construction documents have been completed. Application forms will only be accepted electronically. The “fillable PDF” Application form can be downloaded from the Forms section of this site or by clicking on the above heading (the latest version of the freely available Adobe Reader allows applicants to save completed forms for submittal). Forms are to be email to LUCAdmin@mptn-nsn.gov.
Application Documents
- A minimum of three complete, stamped sets of construction documents must be delivered or mailed to the Land Use Commission.
- An Electronic, PDF, version is also required and will be distributed to all Commissioners.
- Additional Application Requirements for:
- Activities Involving Site Development
- Two additional Sets of Construction Documents are required
- Projects with activities within 50 feet of a wetland require a detailed Wetland Assessment.
- Projects with new or impacting existing stormwater systems require a detailed Stormwater Management Plan (incl. calculations).
- Site developments greater than one acre require a Stormwater Pollution Prevention Plan.
- Activities Within the Resort
- One additional set of Construction Documents is required for the Gaming Enterprise
- Activities Involving Food Service
- Require submittal of a Restaurant Plan Review Submittal Package
- One additional set of Construction Documents is required for Food Safety and Sanitation
- Activities Involving a Salon
- Require submittal of a Salon Registration Form
- One additional set of Construction Documents is required for Food Safety and Sanitation
Application Fee
- 1% of the Total Cost of the Activity for Non-Residential Projects (minimum fee is $50)
- ½% of the Total Cost of the Activity for Residential Projects
